New Client Information

    • Prospective new clients can schedule an appointment over email, text, phone call or through the convenient online scheduling integration on this site.

      1. Call Us

      2. Text Us

      3. Email Us

      4. Schedule an Appointment

    • Clients who schedule online will be contacted by someone from our admin team to help answer questions and provide detailed information prior to the first appointment.

      • It is suggested that new clients book 2-3 appointments ahead in order to be seen regularly without interruption to services.

    • Clients should expect to receive an invitation to the client portal within 24 hours of scheduling their first appointment.

      1. This email will be titled “Resolve has invited you to Join the Client Portal”. Our client portal is powered by Theranest and clients only use this program to fill out intake and consent forms.

      2. These forms consist of a standard privacy practices and informed consent form, the applicable intake packet (individual, couple, child, etc.), a telehealth consent form, and additional consent forms as needed.

      3. To best prepare their therapist, clients should aim to complete intake forms electronically within 48 hours of their appointment.

        1. If an appointment is scheduled less than 48 hours in advance, please complete these forms 12-24 hours in advance.

      4. To submit these forms as smoothly as possible, please fill in all blank text boxes, even if you do not have an answer to a particular question.

      5. HSA, FSA, debit, or credit card information is required to be kept on file for all clients, including those who pay with cash or check.

      6. If forms are unable to be completed electronically, physical forms can be provided on site.

      7. NOTE: Once clients have completed their forms, their portal account will be archived unless further forms are needed. Scheduling and billing management is completed in a different program.

    • If the parents of an adolescent client are not together but share custody, intake and consent forms will be sent to both parents. If a parent has sole custody of an adolescent client or parents are together, forms will be sent to one person.

    • Clients will periodically receive emails, texts, and/or phone calls prior to their appointment to provide customer support and reminders of their appointment(s).

    • Unless intake and consent forms are being completed in person, clients don’t need to arrive more than 5 minutes prior to their start time.

    • All locations have a contactless lobby. Clients check in by navigating to the iPad and typing in their name and their therapists’ name. The therapist will then receive an email that you have arrived and greet you at the start of your appointment.
